fix(provider): extract OpenAI-shaped error messages

Port of upstream anomalyco/opencode#36411.

Replaces the broken body.message || body.error || body.error?.message chain (which rendered [object Object] for OpenAI-shaped error envelopes) with type-safe extraction that walks nested error objects.

Verified: bun test test/provider/error.test.ts and bun typecheck from packages/opencode. Applied cleanly.
