GPT-5.4 has issues with the SQL todo list #1909

Describe the bug
It keeps getting confused on how to call it, it will fail, and then it convinces itself its done once before so everything is fine. It convinces itself everything is fine far too much really, but this one causes it to think it's done things it hasn't. Because it assumes it must have.

Steps to reproduce the behavior
Use GPT-5.4 to plan a multiphase, multistep plan. Instead of producing todos it will try to track its work in the plan.
Expected behavior
It should produce a todo list, and then execute against it.
